Survey Question Tooltips

We have a few questions that could benefit from a little bit of extra information or caution. It would be great to have a function where we can add a small message that pops up as a tooltip when the answer box is selected, or smaller text underneath the main question, or a superscript notation that cites text at the bottom of the survey, or whatever the solution ends up being.
e.g., a service we offer is security testing of wireless network infrastructure; most of this can be done remotely, but there are a couple of survey options that require an on-site visit and associated T&E. We can currently mention this in the survey section description, but it looks a bit messy and might be missed.
